COVID-19 Vaccine Information

Lakeview Clinic been approved as a registered vaccine administration site. We are receiving a limited supply of vaccine and are scheduling appointments for Lakeview Clinic patients that are 16 years of age or older for Pfizer, and 18 years of age or older for Moderna. The type of vaccine fluctuates based upon availability in Minnesota. Moderna requires two shots with the second dose given 28 days following the first dose. Pfizer requires two shots with the second dose given 21 days following the first dose. We will automatically sign you up for your 2nd dose after you are registered for your 1st dose of the vaccine. Your second dose will be scheduled at the same time of the day as your first dose.

Lakeview Clinic is only making COVID-19 vaccine appointments online and is not keeping a waiting list of patients interested in the vaccine. If you check the online schedule and can’t find any available appointments, then we are currently fully booked for our supply. Please check back with our website regularly for any changes in this status.

We appreciate your patience as we work with Carver County and the MN Department of Health to provide you continuous access to the COVID-19 vaccine.

Current criteria for eligibility for a COVID-19 vaccine:

  • Must be a patient of Lakeview Clinic
  • Patient must be 18 years of age or older to receive Moderna vaccination.
  • Patient must be 16 years of age or older to receive Pfizer vaccination.

In order to receive a vaccine, you may NOT have had the following:

  • A Steroid injection 2 weeks before or 2 weeks after COVID vaccines
  • Another vaccine administered within 2 weeks before or 2 weeks after COVID vaccines
  • Some medication allergies may need to be evaluated prior to receiving the vaccine

For the ease of administration of the vaccine, we ask that you wear a short sleeve shirt. This will allow our vaccinators to efficiently provide your vaccination.

COVID-19 Vaccine Phases

Governor Tim Walz and Minnesota Department of Health (MDH) have implemented a phased approach to the release of the vaccine to qualified people. This phased approach takes into account age and health status to define risk levels. Lakeview Clinic is following the phased vaccine schedule, and will only be vaccinating qualified persons at this time. The timeline below outlines the anticipated prioritization schedule for the COVID-19 vaccine.
