COVID-19 Vaccine Information

Vaccines are currently being administered out of our Waconia location only.

Lakeview Clinic been approved as a registered vaccine administration site. We are receiving a supply of vaccine and are scheduling appointments for Lakeview Clinic patients that are 12 years of age or older for Pfizer, and 18 years of age or older for Moderna. We currently only have Pfizer in our vaccine inventory. Moderna requires two shots with the second dose given 28 days following the first dose. Pfizer requires two shots with the second dose given 21 days following the first dose. We will automatically sign you up for your 2nd dose after you are registered for your 1st dose of the vaccine. Your second dose will be scheduled at the same time of the day as your first dose.

Lakeview Clinic is accepting phone call scheduling for COVID-19 vaccine appointments on Wednesdays each week. You may also receive your COVID-19 vaccine during your appointment with Lakeview Clinic throughout the week. Please ask your nurse or provider about the COVID-19 vaccine during your appointment with us!

Current criteria for eligibility for 1st and 2nd doses of COVID-19 vaccine:

  • Patient must be 18 years of age or older to receive Moderna vaccination.
  • Patient must be 12 years of age or older to receive Pfizer vaccination.

Current criteria for eligibility for 3rd dose of COVID-19 vaccine:

Lakeview Clinic is currently requiring that all booster shots be approved by your primary provider. Your provider will evaluate if you meet the following conditions to receive a booster injection for COVID-19:

Those currently eligible for a third COVID-19 vaccine dose include certain individuals with weakened immune systems who meet the high-risk requirement, including patients that:

  • Are receiving cancer treatment for tumors or cancers of the blood
  • Received an organ transplant and are taking medicine to suppress the immune system
  • Received a stem cell transplant within the last two years
  • Have moderate or severe primary immunodeficiency (e.g., DiGeorge syndrome, Wiskott-Aldrich syndrome)
  • Have an advanced or untreated HIV infection
  • Are taking medicine that may suppress immune response

In order to receive a COVID-19 Vaccine, the following conditions must be met:

  • No Steroid injection 2 weeks before or 2 weeks after COVID vaccines
  • Some medication allergies may need to be evaluated prior to receiving the vaccine

COVID-19 Vaccine Eligibility

Governor Tim Walz and Minnesota Department of Health (MDH) have implemented a phased approach to the release of the vaccine to qualified people. At this time, all patients 12 years of age or older are eligible to receive the vaccine.

  • Patients 12+ may receive the Pfizer vaccine.
  • Patients 18+ may receive the Moderna vaccine.
