Appointments are taken during clinic hours. When making your appointment, please be specific about the reason for your visit, so we may schedule adequate time. Please have your insurance information available. Please bring all current medications with you to the clinic at the time of your appointment. If you should be unable to keep an appointment, please notify the clinic as soon as possible. For appointments scheduled more than two days in advance, we will send out a text reminder of your appointment.
Please register at the reception desk each time you visit the clinic. Also please present all insurance identification cards to the registration clerk. Co-pays are due at the time of your visit. Failure to identify your insurance could result in out-of-pocket expenses for you. After registering, you will be directed to the appropriate location within the clinic for your visit.
Urgent medical questions will be transferred to trained nurse triage staff who will answer your questions or further direct you. Routine questions may be answered by a return call from a physician or nurse. Please call back if your call is not returned in a reasonable amount of time.
Test results are often available at the time of your visit. If not, most results are returned within one week. If you do not hear from the clinic in one week, please call us and ask for your test results. Do not assume test results are normal if you have not been notified.
For prescription refills please call your pharmacy.
For life threatening problems, dial 911. Physicians are available for other urgent concerns and may be contacted through the answering service by dialing 952-442-4461.
